On September 9, 2026, the U.S. Department of the Treasury announced that FinCEN had identified approximately $17.5 billion in suspicious financial activity potentially linked to health care fraud. FinCEN’s Financial Trend Analysis reviewed 5,702 Bank Secrecy Act reports filed between March 1, 2025, and February 28, 2026, covering both completed and attempted transactions.

Health care fraud can include billing Medicare, Medicaid, or private insurance for services that were not provided, were not needed, or cost less than the amount billed. Fraudsters may use stolen provider information or fake businesses to receive the payments and then quickly move or spend the money. The analysis found that most of the identified subjects were located in the United States and concluded that home health care businesses were the biggest offenders. Other commonly identified providers included hospice companies, mental and behavioral health or addiction treatment providers, medical equipment providers, and adult or child daycares.

The Treasury is encouraging whistleblowers to report credible information involving fraud, money laundering, and sanctions or tax-law violations. These tips, together with financial institutions’ BSA reporting, can help Treasury and law enforcement identify criminal networks, recover taxpayer funds, and hold scammers accountable.

CCI recommends sharing this update with your BSA officer, reviewing FinCEN’s March 2026 health care fraud advisory, and incorporating applicable red flags into training, monitoring, investigation, and escalation procedures. When filing a SAR on activity related to that advisory, FinCEN requests that you use the key term “HCF-2026-A001” in SAR field 2 and the narrative, select the Healthcare/Public or Private Health Insurance checkbox, and make any other applicable selections.
